I have carried a dining card with me for years.   I mention it in most of my dining blogs.  It was always just a word doc that I put about 8-10 on a page and cut them into slips.  Nothing fancy.  However recently I just had the text added to my business card.

Here is how mine reads.

I am a Celiac with Multiple Food Allergies.
I must avoid the following:

Gluten, Wheat, Rye, Barley, Flour, Oats
Pork, Some Fish & Shellfish
Blue Cheese, Milk   *I can tolerate most
hard cheeses and sour cream
Melon, Cantaloupe, Zucchini, Squash
Gourd, Pumpkin
Legumes: Soy, Beans, Peas,
Peanut, Cashew
Anything Autolyzed or Hydrolyzed, MSG
Sulfites (Sulfur Dioxide,Sodium Sulfite,
Bisulfite, Metabisulfite)
“Natural” and Artificial Flavors, Malt

Many of these items are hidden in
seasonings, sauces,and dressings.

If there is any chance the above
ingredients (even in small quantities)
are in the dish I have selected,
please stop and advise,  I will be
happy to choose another dish.
